Medications at School

 

            In order to provide a safe environment for our students, a few guidelines have been established. (1) Students are not allowed to keep any medication (prescription or over-the-counter) with them. All medication should be brought to the nurse’s office immediately upon arrival at school. (2) No medicine will be given without parent’s written permission. The following information needs to be included in the permission notice:  child's name, physician's name, name of the medication, time to be administered, dosage, possible side effects, and termination date for administering the medication. One permission slip will suffice for the entire year for medications that will be given on a long-term basis or for those medications that must be administered in case of exposure to allergens (unless the medication or dosage is changed by the physician.) Permission forms are available from the school nurse.

(3) All medications must be in the original container. Please do not send medication in zip lock bags, envelopes, etc. (4) The nurse will keep a record of the medications given. All controlled drug prescriptions will be counted weekly and a thirty-day supply is required.
 

Parents should reclaim any unused medications within one week of the termination of the treatment or within one week of the last day of school.  The school will destroy any unused medications after this time.

The school district personnel will not be responsible for any adverse drug reaction when the medication has been given in the manner prescribed.
